Print and complete the following forms:
The night of seminar you may go ahead and make your appointments with the physician, to take your Computer Behavior Assessment & Psychological appointments. After you have attended seminar and have turned in your questionnaire, someone will get intouch with you within 1 week from the date you attended seminar to inform you of what other requrements are needed.
